CALIFORNIA LICENSING

California's Dental Board no longer issues temporary licenses. However, we have had no shortage of non-California-doctors going through the process to take our courses.

If you have a current license issued by another state to practice dentistry that has never been revoked, suspended, or otherwise restricted, AND you have at least 5 years of consecutive clinical practice in the U.S. in the last 7 years and are applying for 'licensure by credential,' you will want to do the following:

1. Complete the Application: http://www.dbc.ca.gov/formspubs/app_lbc1.pdf

2. Complete the Certification of Clinical Practice of Dentistry; multiple forms may be needed if you have been practicing in different locations or for different employers: http://www.dbc.ca.gov/formspubs/app_lbc3.pdf

2a. If you are self-employed, please provide page 1 only of Schedule C or 1120S for at least 5 of the last 7 years, the years you are using to qualify.

3. Complete fingerprinting by fingerprint cards. Please let us know if you would like me to mail you fingerprint cards to have this done in another state.

4. Request official license certifications from each state in which you have ever held a dental license. These can be mailed directly to the Dental Board of California at the address in my email signature, or they can be mailed to you and included with your application.

5. Request a letter from WREB stating that you have not failed the clinical examination within the last 5 years. You may contact WREB at (602) 944-3315 or www.wreb.org to request this letter. This is required even if you have not taken the WREB examination. If you have taken and passed the WREB examination you may want to look into California's licensure by WREB pathway.

6. Provide copies of certificates showing at least 50 continuing education units earned in the last 2 years (prior to the date that your application is received).

7. Provide a copy of the certificate for the California Dental Practice Act continuing education course. They can be taken on line by elite continuing education. https://dental.m.elitecme.com/CA (The study material is attached below.)

8. Provide a copy of the certificate for the California Infection Control continuing education course. They can be taken on line by elite continuing education. https://dental.m.elitecme.com/CA (The study material is attached below.)

9. Provide a copy of the front and back of your Basic Life Support card with signature. This card must be issued by either the American Heart Association or the American Red Cross.
